5xWhy method
The 5xWhy method is a simple technique of getting to the causes of a problem by asking "why?" several times.
Definition
The method helps to go below the surface of symptoms and check what mechanism maintains the problem. The number five is indicative, not magical: sometimes three questions are enough, and sometimes a deeper systemic analysis is needed. It works best with problems that can be described by specific events.
Key ideas
- Przyczyna zamiast objawu
- Sekwencja pytań
- Hipotezy do weryfikacji
Practice and life
Write down the problem in one sentence, ask the question "why?" for each answer and for the last conclusion, add data that confirms it.
Common misunderstanding
It is a mistake to focus on the fault of a specific person instead of the mechanism. The second mistake is to accept the answer as fact without checking it.
Questions for self-reflection
- Dlaczego to się wydarzyło?
- Czy odpowiedź opisuje mechanizm czy winnego?
- Jakie dane potwierdzają ostatnią przyczynę?
